needs of both the client and the family and links clients to needed
services,
resources, and opportunities.
When
necessary, the case manager arranges, coordinates, evaluates and
advocates
for a plan of services to meet each client’s and family’s multiple
needs.

do I contact
my Case Manager?
Always make sure you have your case manager’s phone number so that you may call him or her when you need help. If you call and he or she is not in, leave a message or ask for your case manager to be paged. If he or she is on vacation or off for the day, please ask to talk with the case manager covering for your assigned case manager.
